Publicly funded programs are designed for people that financially qualify for services to get the support they may need.  In Racine County there are two publicly funded long-term care programs that are designed to assist persons with disabilities and/or age related issues in remaining in their homes and to remain as independent as possible. To access either of these programs one must qualify functionally and financially.  The ADRC's Information and Assistance Specialists are your first point of contact to access either of these programs in determining if you or your family are eligible. 

The eligibility assessments that are performed include a functional assessment to determine what care a person needs in their home and their daily life, and a financial assessment to determine if a person meets financial qualifications since these are publicly funded programs. 

If you are unsure of which program would be most appropriate, options counseling is available through the ADRC to facilitate a decision about what services would be most appropriate for you, or your families needs.  Some people wish to private pay for care.  Please click private pay to learn about this option.

Family Care

Wisconsin Family Care Program offers health, home and community services that meet the unique needs of seniors and people with physical and developmental disabilities to remain as independent as possible in the community.   These three types of people are defined as: frail older adults 65 years or older; people with physical disabilities 18 and older and people with developmental disabilities 18 and older.  Some of the services that are offered include: personal care, homemaking and chore services, and long-term care services. 

Partnership

Wisconsin Partnership Program offers an alternative to nursing home care for people in Racine County.  This program provides a full range of long-term, primary, acute and preventative care delivered through interdisciplinary teams.  Participants are able to choose from community based physicians from a list of selected health care providers.  Participants receive all inclusive health care, supervised by an interdisciplinary team, while continuing to live in their homes.   Some of the  services include: home visits by nurses and social workers, medication management, personal care, rehabilitative services, adult day services and home care.
